Centerville Clinics - Uniontown Family Doctors

The Fairchance Office joined the Centerville Clinics network in 1994, helping to provide total health care to the entire community. Centerville Clinics is a Federally Qualified Health Center (FQHC), a non-profit community based health organization. An FQHC provides comprehensive primary care, dental and mental health services to people in all spectrums of their life. Centerville Clinics also assists patients to access specialty and hospital care services not available at the center. Centerville Clinics provides total healthcare to the entire community through a network of 11 community medical, dental, and behavioral health offices serving Fayette, Greene, and Washington counties.

Centerville Clinics is a nonprofit Federally Qualified Health Center (FQHC) dedicated to providing total health care for the entire community. Serving Fayette, Greene, and Washington counties, Centerville Clinics offers comprehensive medical, dental, and behavioral health services to patients across southwestern Pennsylvania, regardless of insurance status or ability to pay. Our story began in the early 1950s, when Joseph A. Yablonski, an International Board Member of the United Mine Workers of America (UMWA), recognized a significant barrier facing coal miners in southwestern Pennsylvania. Although miners were eligible for medical and hospital benefits through the UMWA, many couldn’t access those benefits due to a lack of local physicians. Yablonski envisioned a solution: bring more doctors (especially specialists) to rural mining communities.

Centerville Clinics is a nonprofit health center network serving communities throughout southwestern Pennsylvania. Since 1955, we’ve provided high-quality medical, dental, and behavioral health services to individuals and families—regardless of income or insurance. With twelve medical, five dental, and ten behavioral health sites across Fayette, Greene, and Washington counties, we make it easier for patients to access the care they need, close to home.
