Uniontown Family Doctors opened its doors in 1998 as part of the Centerville Clinics network that provides total healthcare for the entire community.

Carmichaels Clinic has been part of the Centerville Clinics network since 1961. Mr. Yablonski personally picked the location for this new clinic. Centerville Clinics is a Federally Qualified Health Center (FQHC), a non-profit community based health organization. An FQHC provides comprehensive primary care, dental and mental health services to people in all spectrums of their life. Centerville Clinics also assists patients to access specialty and hospital care services not available at the center. Centerville Clinics provides total healthcare to the entire community through a network of 11 community medical, dental, and behavioral health offices serving Fayette, Greene, and Washington counties.

Centerville Clinics provides high-quality, comprehensive healthcare to 40,000 patients Fayette/Greene/Washington Counties. We operate 11 medical, 5 dental, and 10 behavioral health sites located in the tri-county area. We also offer specialty medical care, laboratory, X-ray services, and a black lung clinic program, for those with pulmonary diseases.
