Founded in 1973, it is nationally approved by the Continuing Care Accreditation Commission. Its residential accommodations include two-bedroom brick villas, studios, and one- and two-bedroom apartments. Glacier Hills Care and Rehabilitation Center provides 24-hour skilled nursing services, including rehabilitative and short-term care. Its services also include fall prevention, wound care and behavioral management programs, pain management, bladder-control training, psychiatric services, continuing education, and dental, podiatry and ophthalmology services. The organization s life care community offers short-term assisted living, skilled nursing and rehabilitation services. Additionally, it offers a variety of services and facilities, including concierges, a gift shop, beauty salon, barbershop, library, chapel, wellness center, shuttle bus, housekeeping, laundry, on-site banking and maintenance.

Addison Heights Health and Rehabilitation Center is a place to get better, a place for living. We are a 90-bed skilled nursing center, and a proud member of the Consulate Health Care family. Opened in 1960, we have 15 private rooms, 38 semi-private rooms, and can provide accommodations for couples that become residents of our care center. Addison Heights also offers features such as an outdoor recreation area, garden and patio, a rehab gym, a beauty/barber shop, pastoral support, and transportation services.
