Established in 1981, Home Health & Hospice Care, also known as 3HC, is one of the leading health care providers in North Carolina. It is a community-based, nonprofit organization that is certified by Medicaid and Medicare. The organization s programs are accredited by the Community Health Accreditation Program (CHAP). It staffs more than 400 professionals and operates a hospice center and over five clinical offices. The organization offers skilled nursing and respite care services and conducts physical, speech and occupational therapies.
