Our team empowers individuals with knowledge and life skills to make effective behavior changes that address the underlying causes of disease. Services Offered include individual medical appointments, shared medical appointments - a physician and a behavioral health provider meet with a small group of patients to encourage questions and sharing of concerns and experiences; providers develop a unique care plan for each patient; health coaching; medically supervised exercise; Complete Health Improvement Program (CHIP); group nutrition and cooking classes; individual nutrition consults.
