Hello, I'm Amy Poliak, a pediatric nurse practitioner for Affinity Medical Group. While treating pediatric patients, my goal is to build relationships with families and help my patients develop a healthy lifestyle early on. I have a special interest in sports medicine/concussion care;and adolescent health, and look forward to working with new parents and patients. Outside of the office I enjoy running, skiing and camping.

Cassandra Toeppler, APNP, is family practice nurse practitioner with Ascension Medical Group. Toeppler is board-certified in family practice medicine and provides comprehensive care to people of all ages including patients suffering from chronic and acute health conditions such as depression, anxiety, ADHD, arthritis, diabetes, COPD and cardiovascular disease. Outside of the office, Toeppler enjoys spending time with her family, outdoor activities, attending athletic events and reading.
