Lakeland Care, Inc. is a private, non-profit organization based in Oshkosh, WI, dedicated to empowering individuals, strengthening communities, and inspiring futures through person-centered managed care services. With over two decades of experience, Lakeland Care offers the Family Care program to eligible elders and adults with disabilities, helping them live independently at home and in their communities.

As a proud steward of Medicaid dollars, Lakeland Care partners with over 1,000 local providers across 22 counties and two Tribes in Northeast and North Central Wisconsin.
