The Division of Quality Assurance is a part of the Wisconsin Department of Health and Family Services. The division, also known as DQA, provides regulation and licensing of more than 40 programs and facilities in Wisconsin. It serves the providers of long-term care, health, mental health and substance abuse and caregiver background checks and investigation services. The Division of Quality Assurance regulates adult family homes, ambulatory surgical centers, home health agencies, hospices, hospitals, laboratories, nursing homes and outpatient rehabilitation centers. The division publishes the Provider Overview Report, which provides information to the staff who survey short- and long-term health care providers.
