Sarah Meyer is a board-certified advanced practice nurse prescriber. She obtained her BSN earned her DNP-Family Nurse Practitioner program from the University of Wisconsin in Oshkosh, WI. Sarah’s medical interests include primary care, patient education, and addiction and substance use disorders. Sarah is a member of the American Academy of Nurse Practitioners and the Wisconsin League for Nurses. She is board certified by the American Nurses Credentialing Center. In her free time, Sarah enjoys spending time with her children, attending their sporting and academic events, walks with her husband, yoga, and volunteering at church.

With more than 30 years of experience, Belmont Nursing and Rehabilitation Center provides health care services for senior citizens. It is a 90-bed facility that offers long- and short-term care options for individuals who need nursing and rehabilitative care services after surgery and illness. The center additionally offers treatment services for senior citizens with Alzheimer s disease. It also provides a range of rehabilitative solutions, such as therapy management, respite and hospice care, and physical, occupational and speech therapy services.
