In order to begin Physical Therapy treatment at Salem Physical Therapy, you may call our office at 540 ) -302-2255. Please notify the receptionist which location you prefer to be seen. It is important to first accurately complete all registration forms to ensure we have the correct demographic and insurance information on file for you. You may download and complete the forms below prior to your arrival for your first appointment and bring them with you, or fax to ( 540 ) -302-2256. Otherwise, plan to arrive 15 minutes prior to your appointment time to complete the paperwork in the office. For Medicare patients, there is an additional packet of papework that must be completed prior to the first appointment. We are happy to mail this to you in advance or you would need to come approximately 45 minutes early to your appointment to complete it. Signed or verbal consent to treat with a Salem PT staff member if the patient is under the age of 18 Your first appointment is known as the initial evaluation. You can expect this appointment to last approximately 45-60 minutes. During this appointment, you will be seen by a Doctor of Physical Therapy who will evaluate your injury based on the physician referral and the information you provide on the history form. It is helpful to dress or bring appropriate clothing ( i.e. sweatpants, t-shirt, shorts if being seen for a lower extremity injury ). They will assess your posture, alignment, range of motion ( ROM ), flexibility, strength, proprioception/balance, gait, running, etc'Based on your clinical findings and reported symptoms, you will undergo a treatment that may involve manual therapy or modalities ( heat, ice, e-stim, ultrasound, iontophoresis, traction, taping ) as appropriate and you will be given and individualized home exercise program ( HEP ). There will be ample time to discuss your injury and answer any questions that you may have. At the conclusion of your appointment, you will schedule follow-up visits following the referring physician's orders or physical therapist's recommendations.
