Sentara PACE - Churchland, a Program of All-Inclusive Care for the Elderly is located in the Churchland area of Portsmouth. The facility serves the Chesapeake, Portsmouth and Suffolk communities. This program is a comprehensive health care and supportive services program for frail seniors who wish to remain in their homes and community. It is an alternative to nursing facility eligible individuals, but who would prefer to remain at home with services coordinated by Sentara PACE. This facility located at Churchland Shopping Center in Portsmouth provides transportation, meals, medical care and socialization for seniors living with their families, who qualify for nursing facility level of care.
