Sentara PACE - Norfolk - a Program of All-Inclusive Care for the Elderly is located in Norfolk. The facility serves the Virginia Beach, Chesapeake and Norfolk communities. This program is a comprehensive health care and supportive services program for frail seniors who wish to remain in their homes and community. It is an alternative to nursing facility eligible individuals, but who would prefer to remain at home with services coordinated by Sentara PACE. This facility located in Norfolk provides transportation, meals, medical care and socialization for seniors living with their families, who qualify for nursing facility level of care.
