Welcome to Our Lady of the Meadows - senior living & memory care. We are level III Community Care Homes with a passion for elder care. When being alone or with family who work outside of the home becomes unsafe, lonely, or both, having to choose alternative living accommodations can be overwhelming. We can answer questions, provide companionship, introduce you to other residents who have had similar concerns, and make the process as easy as possible. Established in 1963, Green Mountain Nursing Home is a 70-plus-bed facility located in Colchester, Vt. It is a part of the Brookside Alliance, a family-owned and operated health care organization. The facility is licensed to provide intermediate care to patients with acute and long-term illnesses. Green Mountain Nursing Home offers short-term medical treatment, traditional long-term skilled nursing care and comprehensive rehabilitative services. Its medical services include dialysis care, and pain and diabetes, cardiopulmonary and tracheotomy management. The facility s rehabilitation staff offers physical and occupational therapies and speech and language pathology services for neuromuscular disorders. Its staff includes primary care physicians, social workers, therapists, registered dietitians and licensed nurses. Green Mountain Nursing Home also offers case management services that include clinical assessment, discharge planning and family education.
