Jordan B. Koncinsky

Kyle Bradford Jones, MD, FAAFP is a board certified Family Physician. He received his bachelor’s degree in liberal arts and sciences from Utah State University, and obtained his medical degree from the Medical College of Wisconsin in 2009. Dr. Jones completed his residency training in Family Medicine at the University of Utah. His clinical practice is at the Neurobehavior HOME Program, a patient-centered medical home that cares for individuals with developmental disabilities, where he leads the Primary Care Team. In his leisure time, he enjoys participating in sports, reading, watching movies and spending time with his wife and four children.

Tyson C. Schwab, MD, MS, attended the University of Utah School of Medicine and trained in Family Medicine at Intermountain Healthcare where he served as Chief Resident. He has completed additional graduate education and training in bioengineering at the University of Utah. In 2021, Dr. Schwab joined the University of Utah School of Medicine Faculty. His clinical responsibilities at the University of Utah primarily include the Neurobehavioral HOME program, where he serves as Associate Medical Director. Dr. Carbone is the Medical Director of the Child Development Program within the University Developmental Assessment Clinics, providing evaluations of children with suspected autism and other developmental disabilities as part of a multidisciplinary team. He sees patients with autism and other developmental disabilities for ongoing care of medical and behavioral/psychiatric co-occurring conditions.
