Daniel Broadbent, MD | Farmington Family Medicine

Sarah Gadd was raised in Kaysville and is excited to serve the Davis County community with family medicine and urgent care. She became a Registered Nurse in 2016 where she worked in labor and delivery and at an eating disorder treatment center. Sarah is very passionate about preventative healthcare and believes in a holistic, patient-centered practice. She is interested in all aspects of family practice including women's health, pediatrics, urgent care, mental health, and more.

Traci Allison is a Family Nurse Practitioner at Ogden Clinic | Farmington. She received her Master of Science as an FNP from Weber State University in 2019. Traci’s clinical interests include anxiety, depression, bipolar disorder, well-child care, dermatology, and women’s health. Traci resides in Kaysville with her husband and four children. In her spare time, she volunteers at Seager Memorial Clinic. She also enjoys skiing, quilting, and spending time with her family.

Tonja Smith, FNP-C, is a Nurse Practitioner specializing in Pain Management at Ogden Clinic in Farmington, Utah. She is dedicated to helping patients with chronic pain conditions by building trust, providing compassionate care, and improving quality of life. Tonja earned her Master of Science in Nursing from Maryville University and her Bachelor of Science in Nursing from Western Governors University. Outside of work, she enjoys reading, nature walks, time with her granddaughter, and has a love for chocolate and a good steak.
