Michael Phan, MD

Pablo Lozano, MD, a cardiology specialist, earned his medical degree from The Autonomous University of Guadalajara, Mexico, and completed his internship and residency at The University of Texas Health Science Center at Houston. He completed a fellowship in Cardiovascular Disease at The University of Texas Health Science Center at Houston. Dr. Lozano has been recognized for providing outstanding cardiovascular and stroke care by the National Committee for Quality Assurance (NCQA) and American Heart Association/American Stroke Association (AHA/ASA) Heart/Stroke Recognition Program. He joined Kelsey-Seybold Clinic in 1996.

Yolanda Ajala, MD, earned her medical degree from the University of Texas Medical Branch at Galveston in 2004 and completed her Internal Medicine residency at Franklin Square Hospital Center in Baltimore, Maryland, in 2007. Board certified in Internal Medicine; Dr. Ajala joined Kelsey-Seybold in March 2025. Dr. Ajala's expertise includes patient assessment, inpatient care, treatment planning, preventive care, chronic disease management, and minimally invasive cosmetic procedures. She emphasizes collaboration, quality improvement, and patient-centered communication to improve outcomes and care coordination.

Vivianna Jaclynn Chapa, MPAS, PA-C, attended Texas State University in San Marcos, where she graduated cum laude with a Bachelor of Science. She then completed her Master of Science in Physician Assistant Studies at University of Texas Medical Branch at Galveston, graduating in August 2023. Vivianna seeks to meet, and advocate for, her patients' needs. She provides general obstetric care, including well woman exams and contraceptive counseling, and treats needs associated with menopause, dysmenorrhea, abnormal uterine bleeding, vaginitis, and urinary incontinence. Her clinical interests include preventive medicine as well as patient education and empowerment. She joined Kelsey-Seybold in November 2023.
