Home Health Services are available upon physician s referral. Referrals are based upon the patient s identified care needs, homebound status, physician s order and the type of services required. Recipients of Kentucky Medicaid may access Home an d Community Based Waiver services. These services include assessment, reassessment, case management, homemaker services, personal care services and respite. Certain eligibility requirements must be met to qualify for this program.
