Kelsey Miller, CRNP Kelsey Miller, CRNP, is a certified registered nurse practitioner specializing in family medicine. She earned her Bachelor of Science in Nursing from Carlow University and her Master of Science in Nursing from Chamberlain University. She also completed additional training through the AHN Family Nurse Practitioner Residency Program. She is certified by the American Academy of Nurse Practitioners and is a member of the same organization. Lauren Perlis, MD Lauren Perlis, MD, is a family medicine physician with the AHN Primary Care Institute. She is experienced in managing chronic conditions such as cardiovascular disease, diabetes, hypertension, and hyperlipidemia, with a focus on risk reduction and disease prevention. Dr. Perlis provides a broad range of family medicine services, offering acute care, wellness exams, school and sports physicals, and management of mental health issues like anxiety and depression, as well as musculoskeletal pain. Dr. Perlis earned her medical degree from the University of Rochester School of Medicine and Dentistry in Rochester, New York, where she also completed her residency. She is board-certified by the American Board of Family Medicine and welcomes patients of all ages.

Dr. Chismer is a family medicine physician who cares for patients of all ages. Along with preventive care, he provides treatment for illnesses, injuries, and ongoing conditions. He earned his medical degree from the Medical College of Virginia, Virginia Commonwealth University in Richmond. He completed a residency in family practice at St. Margaret Memorial Hospital in Pittsburgh, Pennsylvania. He is certified in family medicine by the American Board of Family Medicine. Dr. Chismer is a member of the American Academy of Family Practice, American Medical Association, the Pennsylvania Medical Society, and the Allegheny County Medical Society.
