Philadelphia Health Management Corporation is a United Way partner agency. Located in Philadelphia, the agency has been operational for more than 40 years. It offers a wide variety of services and programs, such as behavioral health solutions, continuing education and specialized training services, chronic disease management, criminal justice initiatives and early intervention services. PHMC also offers various specialized services, including emergency assistance and environmental health solutions, family and community services, a community health database, health promotions, HIV and AIDS prevention programs, injury prevention services, skilled nursing facility, children s services, and research and evaluation programs. Additionally, Philadelphia Health Management provides emergency shelters, temporary housing support, social welfare activities and respite care.
