Carol H. Mayfield, CRNP Carol H. Mayfield, CRNP is a nurse practitioner with AHN Primary Care Institute. She specializes in family practice and her clinical interest is in osteoporosis. She is also a Provider for the Osteoporosis and Fragility Fracture Clinic providing care for bone health. Prior to this, Carol was a Primary Care CRNP for 10 years with Family Practice Medical Associates South Carol H. Mayfield did her Bachelor of Science in Nursing at California University in California, Pennsylvania and her Master of Science in Nursing at Carlow University in Pittsburgh, Pennsylvania. She is a Certified Registered Nurse Practitioner. She has a background in both critical care and medical/surgical nursing. Carol has been the recipient of the Cameo of Caring Award and in 2023, the AHN Excellence in Advanced Practice Nursing Award. Carol H. Mayfield believes in a comprehensive approach to the care of patients and sees patients ages 18 and older.

Jarred Holt, DO, is a surgeon with AHN Orthopaedic Institute who specializes in sports medicine and injuries of the shoulder and knee. He performs corrective procedures including articular cartilage restoration and knee ligament reconstruction, with special care toward cartilage preservation. In addition to this he also performs knee and shoulder replacement surgery, rotator cuff surgery, fracture care, and a variety of other procedures for active individuals. Dr. Holt earned his medical degree from Philadelphia College of Osteopathic Medicine in Philadelphia, Pennsylvania and completed his residency at Summa Western Reserve Hospital, Cuyahoga Falls, Ohio. He completed subspecialty fellowship training in sports medicine and shoulder surgery at Tria Orthopedic Center/University of Minnesota. While here he was involved in care of athletes at all levels including, high school, collegiate, and professionals across a wide range of sports. He has provided special assignment coverage to athletic organizations due to his expertise in sports medicine including NCAA and NHL hockey, Major League Baseball, X-games Minneapolis and a number of other community events. Dr. Holt is certified by American Osteopathic Academy of Orthopedic Surgeons and actively performs orthopaedic research related to his specialties.
