Pacific NW Hospice Foundation

Since 1982, Care Partners has provided hospice and palliative care to thousands of individuals and families. We create customized support plans to encompass physical, emotional and spiritual well-being. We help patients and their care givers, healthcare professionals, and with long-term care. Care Partners is also able to support serious illness care from a private home. Care Partners is an affiliate of the Oregon Nonprofit Hospice Alliance. We are a nonprofit organization dedicated to providing quality and compassionate care for patients and families experiencing a life-limiting illness.

Legacy VNA Hospice, located in Portland, Ore., is one of the centers operated by the Legacy Health System. It provides supportive care for adults and children who are terminally ill. The services offered by the center include intermittent visits by nurses and other health care professionals, management of pain and symptom control, medical treatments as prescribed, patient care instruction for family members, personal care, and assistance in obtaining medical equipment, supplies and medications. Legacy VNA Hospice also provides counseling and emotional support for patients and their families, companionship and practical services by lay volunteers and bereavement follow-up for families. The center provides information and guidelines about insurance, financial aid, in-home support, transportation agencies and community agencies. Its staff includes registered nurses, home health aides, therapists, medical social workers, pharmacists, bereavement and dietary counselors and volunteers.
