Welcome to Applegate House of Grants Pass! Our licensed memory care community opened in January 2011 and features 15 private rooms. Our specially trained memory care team is well equipped to care for individuals living with Alzheimer’s and other forms of dementia, while promoting comfort and familiarity in our cozy boutique home.
