Founded in 1997, it is a joint venture between the Hospice of Medina County and Medina General Hospital. It provides pain management and family education and support solutions. The organization offers education on disease treatment and prevention. BridgesHome Health Care also provides assistance with personal care activities. The organization provides Speech, physical and occupational therapies. In addition, it offers hospital, home and nursing facility evaluation services. BridgesHome Health Care also provides case management, nutritional counseling and 24-hour skilled nursing services.
