The bill I received has no billing date or Dr involved. Their address says they are in GA, but with 508 area code on their number which is MA. Called, said I was 7th in line and after 20 minutes...

Never sent me a bill then sends me a bill saying this a "final" notice and if not paid in 10 days will turn it over to collections. It would have been nice to receive a bill before a final bill.
