Liberty Commons Nursing and Rehabilitation Center of Halifax County in Weldon, North Carolina provides a complete continuum of care, including short-term rehabilitation, long-term living, outpatient therapy, skilled nursing care, hospice care, palliative care and respite care. The center is located in the scenic northeastern part of the state and is easily accessible from I-95. When you arrive at Liberty Commons, you are greeted by a clinical team who develops a person-centered care plan designed to help you regain skills and decrease the chance for readmission back to the hospital. You also have access to medical services that address secondary conditions for a complete treatment plan.
