CornerStone FamilyServices Group together with Medicaid enrollees will work to create a person-centered service system focused on recovery and on integrating physical and behavioral health to improve health outcomes. Person-centered means that providers listen to the person receiving care, that they help the person understand their choices, and that people can be in control of their own lives and recovery. CFSG is dedicated to provide: Coordinated Care Integration of physical and behavioral health services Recovery oriented services Patient/consumer choice Protection of continuity of care Ensure adequate and comprehensive networks Address the unique needs of children, families and older adults By doing so we believe that we can overcome the obstacles that are associated with mental health.
