San Simeon by the Sound Center for Nursing and Rehabilitation is a community-based not-for-profit, 120-bed skilled nursing, rehabilitation and adult day care facility located on the storied North Fork of Long Island in the picturesque hamlet of Greenport, New York, along the shores of the Long Island Sound. Simeon provides short-term post-acute rehabilitative care and long-term skilled nursing services, as well as a Medical Model Adult Day Health Care program.
