HistoryFor over a decade, Family Choice of New York has partnered with Independent Health with a Medicare Advantage Institutional Special Needs Plan (ISNP) designed to serve the individuals who require an institutional level of care in Western New York's skilled nursing and assisted living facilities.SpecialtiesOur 'Treat in Place' Model of Care is specifically designed with the goal of keeping the member at his or her current facility as long as clinically appropriate. Family Choice's Advanced Practice Providers work collaboratively with the physician, the facility staff, and the member or their responsible party and is responsible for the planning, delivery, and coordination of care to members across time and setting. Program Benefits include: * Consistent routine assessments by a Family Choice Advanced Practice Provider * Social Workers to assist facility staff with obtaining advance directives * Non emergent transportation (20 one-way trips) * Part D (Medicare Prescription Drug Coverage)
