Seniors, juggling complicated drug regimes and medical issues, also have to deal with increasingly complex health care environment. This includes cost of transportation, prior authorizations, Medicare prescription drug plan, brand versus generic to name a few. Moreover, hospital discharge patients sooner resulting in sicker people at home. Home visit was a natural fit to overcome these barriers. Complete assessment, dose adjustment, compliance with medicines and diet is best delivered in a convenient and comfortable environment.
