Eldersource Care Management Services, established in 1995 and a joint venture between Catholic Family Center and LIFESPAN, provides information and guidance on community living centers and assistance with health plans to senior citizens throughout New York. The services offered by the center include evaluation of individual's financial status, in-home evaluation and geriatric assessment, written plan of care and long-term needs and assistance with implementation of care plan. The additional services offered by the center include referral assistance, recommendation of community options, family counseling and caregiver support, periodic reassessment and updating of care plan.
