Putnam Valley’s primary care office building is closed and all providers will now see patients at Nuvance Health’s primary care office in Mahopac, located at 979 Route 6. Through these efforts, we will be able to provide better patient and staff experience, enhanced physician collaboration and expanded services, including subspecialties and telehealth. We recognize that some of you have future appointments booked in Putnam Valley. Please be assured that all appointments will be honored at this new location.
