We are committed to assist each consumer in achieving their goal of receiving home care that will allow them to manage his or her care and make decisions based on their personal individual needs. All of our consumers, designated representatives, and personal assistants are fully oriented to the program by our staff. We are dedicated to helping you through the process by encouraging your questions and gladly meeting with you in our office or in the comfort of your home. Our team of coordinators, nurses, and friendly administrative staff are experienced and knowledgeable in all phases of home care. We are approved to provide CDPAP services by the New York State Dept. of Health and look forward to helping you through the CDPAP process.
