The Koldin Law Center recommends to its clients that a Health Care Proxy be signed which also makes specific reference to one's wishes regarding feeding tubes and hydration. Also, we suggest that a specific provision be in the Health Care Proxy expressly granting the appointed agent ( s ) authority to obtain your medical records under the HIPAA privacy laws. The standard Health Care Proxy forms do not include specific instructions regarding feeding tubes, hydration and HIPAA.
