Hospice Care Network

Hospice Care Network, also known as HCN is a nonprofit hospice organization that serves families and patients in Nassau, Suffolk and Queens counties in New York. The organization offers nursing homes and assisted living facilities for senior citizens. It operates Hospice Inn, which maintains more than 15 private rooms and is equipped with family-style kitchen and outdoor terrace. Hospice Care Network also offers counseling and medication management services. The organization is a member of North Shore-Long Island Jewish Health System. It is accredited by The Joint Commission. Hospice Care Network is covered by Medicaid, private insurance plans, HMOs and managed care organizations. The organization maintains its corporate headquarters in Woodbury, N.Y.
