In operation for more than 30 years, Cobble Hill Health Center is a nonprofit skilled nursing facility located in Brooklyn, N.Y. It offers a range of residential and community-based health care services for the elderly and people with disabilities. The center s long-term home health care services are licensed by the New York State Department of Health. Cobble Hill Health Center offers heart and blood pressure monitoring, wound care, medication administration, and respiratory, physical, occupational and speech therapies. In addition, it offers a range of support services, such as rehabilitation, medical day care, nursing home placement, enriched housing and 24-hour emergency coverage. The center provides respite care, nutritional and audiology services, as well as serves patients who suffer from Alzheimer s disease and dementia-related disorders. Cobble Hill Health Center also provides palliative and subacute care programs.
