The Community Access Program, also known as CAP includes a range of support services for the developmentally disabled individuals and families of Rutland County, Vt. It offers various residential, case management, career counseling and nursing services. The program is approved by the Commission on Accreditation of Rehabilitation Facilities, also known as CARF. The Community Access Program is administered by Community Care Network, which is a provider of community-based services in the State of Vermont. Community Care Network offers a variety of health, human, education, employment and rehabilitative services. It conducts a community rehabilitation and treatment program that features case management, outpatient treatment, psychiatric evaluation, transitional living and housing assistance services for adults. Community Care Network also provides aftercare, relapse prevention and medication-assisted therapy services through its substance abuse program.

WestBridge Community Services offers medical care and support services for individuals with mental illness and co-occurring substance use disorders. The agency provides nutritional food behavioral care and medication evaluation services. It operates the Assertive Community Treatment program, which assists with issues related to depression, homelessness and legal conflicts. WestBridge Community Services features case management, crisis intervention and vocational training programs. The agency offers individual counseling services that include various techniques, such as motivational interviewing, cognitive behavioral therapy, relapse prevention and psychosocial education. In addition, it provides assistance with daily activities, including dressing, bathing and personal hygiene.
