Community Health & Hospice was founded in 1975 as a visiting nurse organization, known as Lakes Region Community Health Agency. Located in Laconia, N.H., Community Health & Hospice is a private, nonprofit home health and community support service agency serving the residents of New Hampshire. The organization supports and educates families through pediatric visiting nurses, mother-baby postpartum visits, pediatric care management, prenatal home visits, immunizations, child development programs and partners in health. Its young family program serves children suffering from serious illness or developmental disability, premature infants, children with no health care provider and parents with concerns about child development.
