Community Health Access Network

Community Health Access Network (CHAN) is a Health Center Controlled Network (HCCN) in New Hampshire, dedicated to advancing community health through technology, training, and collaboration. Established in 1995, CHAN provides health care IT services, including electronic health records and technical support, to 15 Federally Qualified Health Centers (FQHC) and 3 Healthcare for the Homeless Programs.

With a focus on automating primary care health records, CHAN's fully integrated Electronic Medical Record (EMR) system supports over 130 providers and partner hospitals. Additionally, CHAN offers technical assistance and system services to local collaborators and consultation to peers at a national level, aiming to ensure access to efficient, effective quality health care for all clients in the community, particularly the uninsured, Medicaid, and medically underserved populations.
