Referrals can be made directly to the unit. A Senior Care professional is available to provide a confidential assessment in the home or at our facility.We are pleased to announce that the Family Care Main Street in Collins, and Family Care Clinic of Seminary are now open for business. We are grateful to be part of these towns and are excited to bring another benefit to these two communities. Dr. Ginger Pace-Herndon, formerly of Wesly Medical Center, is your physician at the Seminary clinic. Jamey Shivers FNP, of CCH, is now seeing patients at the Main Street clinic.
