SEMCIL United Home Healthcare Choices (SEMCIL UHHC) is a Medicare-certified agency that offers a variety of home health care services for individuals throughout southeastern Minnesota. The agency provides several services related to skilled nursing, home health aides, companionship, homemaker, and physical, occupational and speech therapies. SEMCIL United Home Healthcare Choices administers cardiopulmonary, diabetes, neurological and orthopedic programs. It also offers programs in the areas of specialty wound, rehabilitation, and medication and pain management. The agency participates in the Medication Options Program (MOP), which is a community-based program that includes medication, symptom and side effect monitoring, and physical and mental health assessment for adults with serious and persistent mental illness. Independent Living Resources (ILR) is a part of the Wisconsin Independent Living Network, dedicated to empowering individuals with disabilities to lead self-sufficient lives in their communities. Through advocacy, education, and choice, ILR offers a range of services including information referral, transition support, advocacy, peer support, and independent living services. ILR serves a 13-county region in Wisconsin, assisting individuals in exploring alternatives to institutionalization and encouraging them to make their own decisions about how they live.
