Habilitative Services

Established in 1995, Oak Hills Living Center is a provider of nursing care services to seniors in the City of New Ulm and surrounding communities. Located in New Ulm, Minn., it offers an array of services, such as assistance with activities of daily living, laundry, housekeeping and maintenance assistance for residents, barber/beautician services, nursing services and care. Additionally, the center provides physical therapy, occupational therapy and speech therapy. Oak Hills Living Center is home to more than 125 seniors in both the skilled nursing facility and assisted living apartments.

The Sleepy Eye Care Center offers skilled nursing and therapeutic care services for senior citizens. The center provides home health and adult day care services. It also offers assisted and senior independent living services. The Sleepy Eye Care Center maintains private and semiprivate rooms. The center offers physical, occupational and speech therapies. In addition, it employs a staff of certified nurses, health care personnel and nursing assistants, as well as a clerk, activities assistant and customer service representative. The Sleepy Eye Care Center accepts insurance plans from several companies, such as Medicare and Medicaid. Located in Sleepy Eye, Minn., the center is a part of Volunteers of America, which provides human service programs to more than 2 million people annually.
