Trust Express Home Health Care to help advocate for your loved one when you can't be there. Our goal is to keep your loved one in the comfort of their home for as long as possible. We cater services to keep them as independent as possible. We will meet to assess their functional, cognitive, emotional and social needs to get a comprehensive picture of the member's needs and desires. Once the assessment is complete, we generate an individual plan of care to meet the member's needs. Express Home Health Care will help coordinate, arrange and monitor the assistance needed by a Personal Care Assistant or family member who may be employed through our agency.
