If you have an urgent medical problem and need assistance when the office is closed, please call the office and your call will be directed to the Trinity IHA Call Center. The call center staff will assist you and contact the physician on-call to address your concerns. We encourage you to sign up for the Trinity Health MyChart patient portal, which offers patients personalized and secure online access to portions of their medical records. Trinity Health IHA Medical Group, Primary Care - Schoolcraft Campus is a designated Patient-Centered Medical Home (PCMH). PCMH patients and their families (when appropriate) establish a partnership with their practitioners to ensure they have the support needed to make decisions and participate in their own health care. As a practice, we know that preventive care, including vaccinations, is the most important part of our service to patients and their families. To best protect your children, our other patients, as well as our staff and providers from vaccine-preventable illness, we have made the decision to only accept families into our practice who plan to fully vaccinate their children according to the CDC, American Academy of Pediatrics and American Academy of Family Physicians guidelines.
