Established in 1895, Visiting Nurse Association of Western Michigan provides a variety of home-based services and located in Grand Rapids, Mich. Its services include cardiac care, skilled nursing services, mental health care and pediatric care. The association s cardiac care programs offers services that include telemetry monitoring, wound care, cardiac assessments, medication education, nutritional counseling, stress reduction, lifestyle modification, exercise program development, and physical and cardiac rehabilitation. Additionally, Visiting Nurse Association of Western Michigan offers rehabilitative services for people suffering from physical disabilities or chronic conditions, such as stroke, Parkinson's disease, Multiple Sclerosis, neurotrauma, arthritis, joint replacements, and amputation and orthopedic injuries. Rehabilitative services offered, include rehabilitation nursing, medical social work, home health aide assistance, and physical, occupational and speech therapies. The association also offers hospice services for patients suffering from life-limiting illness and terminal diagnosis. Visiting Nurse Association of Western Michigan is accredited by the Community Health Accreditation Program and is part of Spectrum Health.
