Formerly known as Tri-Valley Elder Services, Tri-Valley is nonprofit organization for senior citizens. The organization specializes in a variety of home care and Meals on Wheels programs. It provides personal care attendant, adult family care and congregate housing programs for younger, disabled individuals. Tri-Valley serves the Bellingham, Blackstone, Brookfield, Charlton, Douglas, Dudley, East Brookfield and Franklin areas in Massachusetts. The organization offers homemaker, chore assistance, money management, and personal and family care services. Its family caregiver support program assists caregivers with information, education, support and services. Located in Dudley, Mass., Tri-Valley Dementia Day Care also serves individuals with dementia from Alzheimer s disease.

Item 4 Description Auburn Youth and Family Services was established in 1976 as the Auburn Project, a non-traditional means to meet the needs of Auburn's youth. The focus of the Auburn project was the prevention of drug/alcohol abuse through education and intervention. Over the years, the programs expanded. In 1991, the agency's name was changed to Auburn Youth and Family Services to reflect this growth and expansion of services. The services of AYFS encompass the treatment of the entire family. The strength of our programs and services is prevention and early intervention with families in need. The mission of Auburn Youth and Family Services, Inc. is to promote the healthy development of youth and families in Auburn.
