Person-Centered Care Coordination Care Alliance of Western Massachusetts (CAWM) is a partnership of eight agencies working together to connect consumers with long-term services and supports. We are a team of compassionate professionals providing person-centered care coordination services for MassHealth ACO and MCO members ages 3-64. Our partner agencies have over 4 decades of experience in Hampden, Hampshire, Berkshire, Franklin, and Worcester Counties.

Charlene Manor Extended Care Facility is an affiliate of Berkshire Healthcare and provides rehabilitation and long-term care to patients. It has a 40-bed nursing unit that provides treatment and support services to individuals suffering from and Alzheimer s disease. The health care center provides various therapies, such as physical, occupational, speech, intravenous and respiratory. Its services also include pain management, wound care, enteral therapy and 24 hour hospice care. The health care center s additional services include psychiatric consultation, and family and group counseling. Charlene Manor Extended Care Facility offers exercise classes, pet therapy, and social and recreational activities and events. The health acre center is located in Greenfield, Mass.
