We provide services in the office, hospital, nursing home, and at a walk-in clinic. We even make house calls to home-bound patients who cannot drive. Prima CARE has grown tremendously since its inception. We now have 160 providers who see patients in Fall River, Somerset, Tiverton, and Westport. Our facilities provide a full range of diagnostic and laboratory services, including MRI, CT scans, nuclear cardiac testing, ultrasound, and bone density testing in our new 60,000 square foot medical facility in Fall River. Dr. Rakesh Gupta is a Board-certified specialist in Sleep Medicine. He is a graduate of the Medical College Baroda and the S.S.G. Hospital in India, where he began his post-graduate medical training. He trained in Internal Medicine in Maryland and Michigan, followed by a Fellowship in Pulmonary & Critical Care Medicine at UConn and a Fellowship in Sleep Medicine at the Mayo Clinic. Dr. Gupta has been an Assistant Professor of Medicine at Boston University, practicing and teaching Pumonary Medicine and Sleep Medicine at Roger Williams Medical Center for nearly 15 years before joining Prima CARE in 2013. He served as Director of the Roger Williams Sleep Disorders Center and has now chosen to devote his clinical pursuits to Sleep Medicine. He is expert in the evaluation and management of the full range of sleep problems. He is experienced in the use of various testing modalities (including home sleep studies) and advanced treatment modalities (such as ASV, iVAPS, and AVAPS) to treat complex sleep disorders.

Dr. Simon Melnick is a Board-certified Family Medicine physician, caring for patients of all ages from adolescents to the elderly. Dr. Melnick attended the Saint Petersburg State Pediatric Medical University and then earned his Doctorate in Osteopathic Medicine at The New York Institute of Technology, College of Osteopathic Medicine. He completed his Family Medicine Residency at Saint Barnabas Hospital in the Bronx. Dr. Melnick has practiced osteopathic and manipulative medicine in Rhode Island for the past 20 years, most recently practicing in Fall River and joining Prima CARE in 2022. He is a member of the American Osteopathic Association and the American College of Osteopathic Family Physicians.
