If your 801101 is not available, you will meet with one of our other 801101s or pediatric nurse practitioners. In 2010, we adopted the Medical Home model of care. The AAP has defined the Medical Home as accessible, continuous, comprehensive, family-centered, well-coordinated, and compassionate care. Our medical home care coordinator leads our efforts to continually improve our patients' experience and the quality of care we deliver.
