Located in Stoughton, Mass., its short-term care unit offers 24-hour care by licensed nurses, access to all long-term care services and amenities, and multidisciplinary discharge planning to ensure a smooth transition to home. Short-term care offered comprises hospice care, respite care, and complex medical and rehabilitation services. Copley at Stoughton s long-term service offers medical, social, cognitive, and rehabilitation programs to patients. Residents at the long-term are unit are also offered services that include therapeutic recreation, consultant services, and continuous monitoring and assessment. Additionally, residents and family members can also access the support of the Social Services Department.
