Joshua D Allen, MD

Sarah L Gehm Coulter, APRN, earned her bachelor’s degree in nursing from Bellarmine University and her master’s degree in nursing from the University of Louisville. Coulter, whose primary practice focus is adult/geriatric, is a member of the American Association of Nurse Practitioners. A Louisville native, she enjoys spending time with her family.

Denisa D. Parker, APRN, earned her bachelor’s and master’s degrees in nursing from the University of Louisville. Parker is a member of the American Association of Nurse Practitioners and the Kentucky Coalition of Nurse Practitioners and Nurse Midwives. Her practice focus is family practice. A native of Louisville, Parker enjoys playing with her children, sports, cheering on the University of Louisville Cardinals and reading.

Lori M. Scales, M.D. earned a bachelor’s degree from Thomas More College, Crestview Hills, Kentucky, and a medical degree from the University of Louisville, where she completed her residency in internal medicine and pediatrics. Dr. Scales is a member of the American Academy of Pediatrics, the American College of Physicians and the Kentucky Medical Association and is board certified in internal medicine and pediatrics. A Kentucky native, she enjoys spending time with her family.
